Job description:
This position provides mobile case management services for previously homeless clients to enable them to stabilize their housing situations and live independently. The Follow up Support Worker works as a member of an agency follow up team. The successful candidate must have a valid driver's licence, a clean driving record, and access to a personal vehicle that can be used for work. A recent police check is required.
Responsibilities:
- Carry out home visits to clients who are at significant risk of homelessness
- Provide case management planning, supportive counselling, crisis intervention, and information and referral services
- Advocate on behalf of clients with landlords, OW, ODSP, and other services
- Enable clients to establish and maintain links with appropriate services including services relating to general health
- care, addiction treatment, mental health care, ID and document replacement services, volunteer opportunities, job placement, and job training
- Provide life skills training in areas including landlord and tenant rights and responsibilities, budgeting, housekeeping, and pet care
Minimum Qualifications:
- Good oral and written communications skills
- Understanding of issues affecting homeless people
- Skills in assessment, case management, advocacy, and negotiation,
- Personal organizational skills and ability to work as part of a team
- Knowledge of community resources and community services
- Post-secondary education in social services or equivalent combination of knowledge and experience
- Access to a personal vehicle that can be used for work
