This video is an overview of the Hennepin County Street Case Management Project. The Street Case Management Project (SCMP) has been operating since January 1, 1996. As of July 1, 1996 it received a grant from the Chemical Dependency Division of the Minnesota Department of Human Services (DHS). Grant funds were awarded to provide innovative services to persons living in Hennepin County who are homeless, chemically dependent, utilizing high levels of detox and Hennepin County Medical Center (HCMC) emergency department services. This is an overview of how the SCMP is providing these innovative services.
